People taking Ozempic are losing muscle mass, and it's freaking them out
Weight on a scale does not distinguish fat from muscle, and that difference changes how people feel day to day.

A lower number on the scale can conceal a frustrating tradeoff: some of the weight lost on a GLP-1 medicine may be lean mass, not only fat. In the reporting, people who were happy to see their weight change also described feeling weak, less steady, or unable to do everyday things as easily as before. That is why a transformation photo can miss the most important part of the story: whether the body still feels capable.
Muscle is not only for athletes. It helps people climb stairs, carry groceries, recover from illness, and lower the consequences of a fall as they age. The concern becomes especially important as use grows among older adults. The report notes that a quarter of GLP-1 prescriptions dispensed in 2024 went to people over 65, a group for whom a loss of strength can have a larger practical cost than a change in clothing size.
The evidence does not say that everyone taking one of these medicines will become weak or that the medicines are a failure. GLP-1 drugs can be effective tools for weight management and can improve health for many people. But weight loss is not one thing. Fat mass, lean mass, energy intake, movement, and functional strength can all move differently, which makes a single target number an incomplete measure of progress.
The durable question is what kind of outcome a person wants to protect while their weight changes. That belongs in an individual conversation with a qualified clinician, especially for people who are older, have other conditions, or notice weakness. The broader lesson is simple: the goal cannot be only to become lighter. It has to include staying able to live in the body that remains.
